Salary Range: $32.
80 - $57.
65 / hour The pay range above represents the lowest possible rate for the position and the highest possible rate.
Factors that may be used to determine where newly hired employees will be placed in the pay range include the employee specific skills and qualifications, relevant years of experience and comparison to other employees already in this role.
Most often, a newly hired employee will be placed below the midpoint of the range.

Responsibilities: Position Overview: Work to maximize and integrate the Enhanced Care Management program for patients who are considered high-risk for medical and mental health issues including but not limited to depression, anxiety, substance abuse disorders, diabetes, and chronic illnesses.
Screen for mental health issues.
Provide appropriate coordination of resources, referrals, and support in an effort to meet the total needs of the patient.
Offer tools to assist patients with successful self-management interventions.
May provide mental health therapeutic services as needed.
Work in collaboration with medical team, community resources, and outside referrals.
Makes recommendations and implements appropriate treatments plans, then monitors progress through follow-up sessions and/or regular meetings.
Serves as a member of interdisciplinary team consulting with physicians, APPs, nurses, psychiatrists, and medical staff.
Ensures the delivery of quality treatment to patients utilizing best practices and standards of care.
The position actively manages and provides outreach utilizing own vehicle to an assigned panel of high-risk patients.
At the direction of the medical team, helps to facilitate the patient’s care including referrals to specialists, hospitalizations, ER visits, ancillary testing, mental health services, social services, and other community resources as needed.
This position will serve the enhanced care needs of our most vulnerable patient population by meeting them in person, via videoconferencing or over the phone.
Qualifications: Required: Master’s Degree in Social Work or equivalent One (1) year experience providing direct service to high-risk patients Preferred: Experience in the continuity of patient care planning Experience with crisis intervention Experience providing case management Bilingual in Spanish Overview: When it comes to quality, we're 5 Star!
