LVN Field Nurse

Responsibilities: - Provide direct patient care in a home health setting - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed - Monitor and evaluate patients' progress and make necessary adjustments to the care plan - Educate patients and their families on managing their health conditions - Collaborate with other healthcare professionals to coordinate patient care - Maintain accurate and up-to-date medical records Qualifications: - Valid nursing license (RN or LPN) - Experience in pain management, dementia care, hospice care, and health coaching preferred - Proficiency in Cerner and ICD-9 coding systems - Strong clinical skills, including IV insertion and inhalation therapy - Excellent communication and interpersonal skills - Ability to work independently and as part of a team - Compassionate and patient-centered approach to care Note: This job description is not intended to be all-inclusive.
