*TITLE: Clinical Care Coordinator I-Specialty Mental Health Services *
*STATUS: Full Time, Bilingual Required, On-Site/In Person *
*BASE RATE: $20/hour + $1/hr Bilingual w/certification*
*JOB OVERVIEW:*
Report to assigned Clinical Services Manager or department designee.
As a member of the SMHS treatment team, the Care Coordinator I will collaborate with the Care Coordinator II and Clinical Services Manager to ensure front office duties and assignments are completed thoroughly and timely to support excellent therapeutic services and contract achievement.
Provides excellent customer service.

Maintains health and safety of the site environment.
*DUTIES:*
1) In collaboration with Clinical Services Manager updates SmartCare intake and assessment calendar.
2) Provides program and agency information as appropriate to walk-ins linking with Care Coordinator II as appropriate.
3) Demonstrate acceptance and respect while providing check-in to client/family upon arrival for appointments.
4) Assist clients to complete initial intake forms, provide and explain required Informing Materials.
5) Provide linkage and collaboration between VCBH Access & Outreach and Interface SMHS treatment team.
6) Download assigned SmartCare reports to treatment team.
7) Review Intake and Assessment Calendar as required to complete client assignments according to contract standards.
8) Engages clients on appointment reminders and schedule changes updating SmartCare.
9) Assists with completion of referral forms to VCBH Psychiatry, Transition of Care, TBS.
10) Uploads documents to SmartCare on behalf of treatment team.
11) Inventories supplies and materials coordinating orders with ICFS headquarters.
12) Maintains lobby, kitchen and common areas in compliance with postings and safety standards.
13) May assist Care Coordinator II with linkage, referrals and system navigation documenting to SmartCare.
14) Complete timecard, documentation and data entry into various systems and platforms thoroughly and timely.
15) Provide feedback and observations on front office workflow to Clinical Services Manager.
16) Demonstrates knowledge of and acts according to Child and Elder Abuse Reporting laws.
17) Participate in staff development and training, weekly supervision and treatment team meetings.
18) Other related duties as assigned.
*QUALIFICATIONS:*
BA/BS in psychology or related field.
One-two years of experience in front office medical, mental health or social service setting.
Bilingual (English/Spanish) required.
Able to work on-site in person, includes several shifts ending at 6pm.
Basic knowledge of child development, youth and families with mental health needs.
Ability to utilize critical thinking skills to improve front office workflows, establish and maintain cooperative relations with a variety of levels of staff, county department staff and the community receiving services.
Must be able to promote and support the team environment and interact productively with various organizational cultures and be able to work independently.
Able to Team spirit, culturally responsive, trauma informed, problem solver, collaborative, inclusive.
Requires excellent communication and presentation skills, knowledge of Microsoft Office Suite Professional, presentation software, and data entry, EHR experience.
Successful background and criminal clearance required and maintained, including but not limited to California Department of Justice (DOJ), Federal Bureau of Investigation (FBI, Child Abuse Index and Department of Motor Vehicles (DMV).
Proof of a valid California Driver’s License, reliable transportation, proof of automobile insurance (listed as covered), pass and maintain driving approval as required by our insurer.
